Nurture Health is seeking a passionate and clinically skilled Patient Admissions Specialist to facilitate patient access to specialist rehabilitation services. Reporting to the Patient Admissions Manager, you will assess referrals, coordinate admissions, and collaborate with healthcare professionals, patients, and families to ensure timely, appropriate rehabilitation care.

Key Responsibilities

• Assess patient referrals and determine suitability for rehabilitation services.
• Coordinate referral, assessment, and admissions processes in line with Nurture Health standards.
• Build strong relationships with referrers, hospitals, funders, patients, and families.
• Collaborate with multidisciplinary teams to support patient-centred care.
• Maintain accurate records and ensure effective communication throughout the admissions journey.
• Represent Nurture Health professionally within healthcare networks and stakeholder engagements.

Requirements

• Registered with SANC as a Professional Nurse, or registered with the relevant HPCSA board as an Independent Practitioner (Physiotherapy, Occupational Therapy, Speech Therapy, or Dietetics).
• Experience in patient referral, assessment, and admissions within a multidisciplinary healthcare environment.
• Strong clinical reasoning, communication, relationship-building, and organisational skills.
• A passion for rehabilitation and advocating for optimal patient outcomes.
