Flexible Locum Social Worker (Growth Opportunity to Full-Time)

Empowered to Thrive is seeking an experienced and self-driven Social Worker to join our growing private practice on a locum basis with strong potential to transition into a full-time caseload.
This role is ideal for a clinician who values flexibility, autonomy, and income growth, and is comfortable starting with a smaller caseload that will build over time.
The Opportunity
You will be based at a rehabilitation facility at Tokai Estate, supporting patients with complex medical needs (neurological, orthopedic, surgical, and respite care).
Your role will focus on:
- Psychosocial assessment and discharge planning
- Supporting patients and families through counseling and mediation
- Coordinating care and connecting to appropriate services
- Reducing barriers to recovery and engagement

- Flexible Structure: You are required on-site daily, but may leave once your patient work is complete

Requirements
- Current SACSSP registration
- Professional indemnity insurance
- Minimum of 3 years social work experience
- Experience in hospital social work and case management
- Training and experience in palliative care (essential)
- Strong report writing and time management skills
- Ability to work collaboratively within a multidisciplinary team
- Willingness to travel for outpatient consultations
Advantageous: Experience in geriatrics, oncology and curatorship
Compensation
- 60% commission on services rendered
- Earnings increase as your caseload grows
